What You’ll Do

  • Craft Personalized Care Plans: Work closely with residents, their families, and healthcare professionals to design and implement care plans that address mental health needs, ensuring a person-centered approach.
  • Manage Complex Cases: Utilize your expertise in mental health to provide high-quality nursing care, managing challenging behavior with compassion and professionalism.
  • Ensure Accuracy: Monitor and document care, ensuring all records are precise, legal, and easily understood by the team.
  • Lead with Confidence: Organize and guide your care team each shift, ensuring everyone is equipped to provide outstanding care.
  • Facilitate Seamless Transitions: Oversee shift handovers, ensuring clear communication and continuity of care.
  • Monitor Health: Perform routine observations and respond to signs of deterioration, ensuring patient safety at all times.
  • Administer Medications: Safely manage and administer medications, adhering strictly to MAR sheets and protocols.
  • Build Relationships: Create strong, trusting relationships with residents, families, and healthcare professionals, addressing concerns with empathy and understanding.
  • Support Mental Well-being: Encourage residents in their daily activities, fostering autonomy while offering hands-on support where needed.
  • Champion Respect and Dignity: Advocate for the individual needs and preferences of each resident, ensuring they are treated with respect.
  • Respond to Feedback: Be proactive in responding to team and resident input, always prioritizing mental and emotional health.
  • Engage Residents: Facilitate social and therapeutic activities that support mental health, helping residents engage with their community.
  • Maintain Safety: Ensure a safe and supportive environment, following health and safety protocols and managing any risks.
  • Act Swiftly in Emergencies: Be prepared to act promptly in emergencies, ensuring the well-being of residents.

About You

  • You are a Registered Mental Health Nurse (RMN) with a valid NMC PIN.
  • You have experience working with patients with complex mental health conditions and managing challenging behavior in a care home or similar environment.
  • You possess strong clinical assessment skills, especially in recognizing changes in a resident's condition and escalating concerns appropriately.
  • You are dedicated to continuous professional development and strive to improve your skills in mental health care.
  • You have excellent communication skills, whether you're connecting with residents, guiding your team, or updating family members, all with empathy, patience, and clarity.
